Registered Nurse (RN) Case Manager to provide telephonic and occasional face-to-face care coordination, guiding members through complex health needs with a holistic, proactive approach.

- Day shift schedule (Mon-Fri) 8:00 am to 4:30 pm in time zone of residence. Occasional late shift rotation until 8:00 pm

What You'll Do- Assess, plan, and coordinate care to support members' overall wellness
- Use clinical judgment and data to identify risks and remove care barriers
- Develop personalized care plans addressing complex and co-morbid conditions
- Collaborate with multidisciplinary teams to optimize outcomes
- Engage members through strong telephonic communication and coaching
What You Bring - REQUIRED- Active, unrestricted RN license (Compact license preferred)
- Willingness to obtain additional state licenses (company paid)
- 3+ years acute care RN experience (med/surg, ICU, or post-surgical)
- Strong assessment, communication, and critical thinking skills
- Travel of less than 5% may be required in the event of clinical audits
Preferred- Case management or home health experience (1+ year)
- Compact RN licensure
- CCM certification
- Proficiency with Microsoft Office tools
- Strong telephonic communication skills
Education- Associate's Degree in Nursing required (BSN preferred)

Pay RangeThe typical pay range for this role is:
$60,522.00 - $129,615.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
