Remote RN Case Manager (Nationwide)100% Remote | Full-Time | Afternoon/Evening ShiftWe're looking for a compassionate and experienced
Registered Nurse (RN) Case Manager to join our team in a fully remote role. In this position, you'll play a critical role in coordinating care, improving health outcomes, and supporting members through thoughtful, proactive case management.
Schedule: Monday-Friday, 10:30 AM-7:00 PM EST (8-hour shift)
Note: Occasional weekend/holiday on-call coverage (currently volunteer-based)
What You'll Do- Assess, plan, and coordinate care for members via phone (and occasional in-person interactions)
- Develop proactive care strategies to improve short- and long-term health outcomes
- Use clinical data, assessments, and tools to identify needs and connect members to appropriate programs
- Address complex medical and social factors impacting health
- Engage members through effective communication and clinical insight
- Ensure compliance with all regulatory and organizational guidelines
What You Bring - REQUIRED- Active, unrestricted RN license in your state (Compact license preferred)
- Willingness to obtain additional state licenses (company paid)
- 3+ years of RN experience (acute care, ICU, case management, discharge planning, or similar)
- Strong communication and multitasking skills in a tech-enabled environment
- Ability to work independently in a distraction-free home office with reliable internet
- Ability to use a computer station with multiple screens, operate multiple programs simultaneously, and sit for extended periods of time
- Less than 5% travel may be required for on-site meetings, training or system updates or to meet licensure requirements e.g., fingerprinting
Nice to Have- 1+ year of case management experience
- Certified Case Manager (CCM)
- Strong customer service mindset and telephonic communication skills
- Proficiency with Microsoft Office and multi-system navigation
Education- Associate Degree in Nursing (required)
- BSN (preferred)

Pay RangeThe typical pay range for this role is:
$60,522.00 - $129,615.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
