Qualifications

• Current Registered Nurse, Social Worker, or licensure in healthcare fields such as Respiratory Therapist or Physical Therapist required.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g. CCM, ARN, ACM).
• Minimum of 2 years of case management or social work experience in an inpatient setting preferred.
• Effective written and verbal communication skills; proficiency in English required, additional languages preferred.
• Basic proficiency in office software (Excel, Word, Outlook) is required.
• Strong organizational, time management, and analytical skills are essential.
• Ability to maintain confidentiality and work cooperatively with others.

Responsibilities

• Participate in pre-admission screenings as requested.
• Conduct thorough initial patient assessments.
• Develop and tailor individualized treatment plans for patients.
• Provide therapeutic interventions appropriate for the patient's age and needs.
• Evaluate and document patient and family responses to treatment.
• Educate patients and families on resources for achieving functional independence.
• Obtain necessary pre-certifications for patient insurance coverage.
