Job Summary: The Care Manager-Registered Nurse (RN) communicates and coordinates health care services for participants in the Program for All Inclusive Care for the Elderly (PACE). Provides evidence based comprehensive care coordination, monitoring, support, health education and coordination of services in the home or PACE Clinic setting. Collaborates and coordinates with the participant, significant others, physician, pharmacist, and other providers in holistic health care coordination, planning and care management provision of care across settings. Arranges and coordinates community resources, services, health promotion and the ongoing evaluation of care delivery to ensure the provision of cost-effective services and appropriate level and combination of care. Collects and synthesizes information from management and research reports. Applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills. Serves as a role model in participant care delivery methods and consistently demonstrates evidence-based clinical practice. Performs other duties as needed.

Education and Experience: Completion of Registered Nursing (RN) program required. Graduate of an accredited institution with a Bachelor or Associate Degree in Nursing required. Minimum two years' experience in Care Management, Primary Care, or Home care required. Experience working with older adults and families, as well as at a PACE Center preferred. Bilingual (Spanish) preferred.

Knowledge and Skills: Basic knowledge of care and utilization management practices and methodologies. Understanding of PACE regulations. Knowledge of aging process and related psychosocial issues. Strong assessment, problem solving, interpersonal and clinical skills. Collaborative and able to work in a multi-disciplinary setting. Able to work independently, synthesize clinical information, and utilize data from a variety of sources. Able to read; write legibly; use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint). Relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in enforcing policies; work calmly and respond courteously when under pressure; collaborate and accept direction. Communicate effectively in English in person, in writing, and on the telephone; think critically; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.

Licensures and Certifications: California Registered Nurse (RN) Licensure required. Basic Life Support (BLS) certification from the American Heart Associate required within 60 days of hire. Valid driver's license required.
