Under the general direction of the Supervisor/Manager of Clinic Operations with clinical oversight from the Director of Clinical Operations, RN: The Team Based Care (TBC) Care Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and evidence-based workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model.

Required Qualifications:
  • Coursework/Training: Graduate from an accredited school of Nursing.
  • Upon hire: Washington Registered Nurse License.
  • 2 years professional nursing experience.


Preferred Qualifications:
  • Bachelor's Degree in Nursing, from an accredited educational institution.
  • 1 year Experience in ambulatory care nursing.
