Job Description

Responsible for the management of the department to ensure Care Management operations and workflow. Provides clear communication across the continuum to improve patient outcomes. Serves as a clinical resource and lends guidance to staff in utilization management, discharge planning and care coordination issues. Works with Care Management Leadership and Administration in the implementation, maintenance and evaluation of an integrated Care Management program. Evaluates and critically analyzes personnel requirements for on-the-job training, in-service education, continuing education, and orientation of newly assigned personnel. Develops staffing plans. Works closely with the Medical Director, Assistant Medical Director and Physician Advisors.

Qualifications

Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's Degree in Nursing or closely related field, Master's degree preferred, and three to five years of previous work related experience. Licensure required as a Registered Nurse by the State of New Jersey or a Licensed Social Worker by the State of New Jersey. Case Management Society of America Certification CCM or ACM preferred.
