We are seeking an experienced
Registered Nurse (RN) Case Manager to support our centralized care management team. This role focuses on population health, chronic disease management, and care coordination within a Patient-Centered Medical Home (PCMH) model.
Your Role & Impact:- Coordinate care for high-risk and complex patients, improving outcomes and access
- Support population health initiatives, including chronic disease management and preventive care
- Use data and performance metrics to identify care gaps and improve patient outcomes
- Assist with transitions of care to ensure continuity and reduce fragmentation
- Address social determinants of health and support underserved populations
- Collaborate with providers, care teams, and external partners to ensure coordinated care
- Maintain accurate, timely documentation and ensure compliance with regulatory standards
What We're Looking For:- Bachelor of Science in Nursing (BSN) degree
- At least five (5) years' clinical nursing and/or case management experience
- At least two (2) years in case management
- Knowledge of medical terminology, legal and technical aspects of a medical records system, HIPAA guidelines, and ICD-10 and CPT codesComputer proficiency, particularly with Electronic Health/Medical Record systems and Microsoft Office (Outlook, Excel, PowerPoint, Word)
- Bilingual in English and Spanish
Desired Qualifications:- Case Management certificate
- Experience in an outpatient setting or Federally Qualified Health Center (FQHC)
- Experience with Epic Electronic Health Record system

The pay range for this role is
$92,977 to
$139,466 on an annual basis.
