The High Fidelity Wraparound Bilingual Facilitator serves as a dedicated care coordinator, helping youth and families navigate complex needs through a collaborative, team-based approach. Bilingual Facilitators lead Child and Family Team meetings and guide the development and implementation of individualized care plans. Working closely with families and community partners, you will help ensure services are coordinated and aligned with each youth's strengths, goals, and needs. ABOUT MONTEREY COUNTY WRAPAROUND

Wraparound is an intensive, community-based, family centered, and goal driven program. The program utilizes a flexible, creative, strengths and values-based process aimed at helping youth (under the age of 21) and families involved with foster care, juvenile justice, or children's behavioral health solve the problems that they are most worried about in their lives. Services include intensive home based and community-based services, intensive care coordination, collateral/individual skill building, and crisis prevention and response. Central to the Wraparound process is the Family Team which is composed of the youth, caregivers, natural support, and formal service providers which provide team-based and sustainable support in a collaborative way.

RESPONSIBILITIES
  • Facilitation of Child and Family Team, which consists of the youth, family, and natural and formal supports
  • Convene the Child and Family Team (CFT), prepare for and facilitate meetings, and update and distribute the Plan of Care
  • Guide the CFT through needs discovery, strengths identification, and incorporation of tools such as the IP-CANS to inform planning and monitor progress
  • Lead the development of the family's vision for success, the youth and family's goals for getting there, and the wrap team's strategies for support
  • Complete clinical documentation including but not limited to assessments, diagnosis, treatment planning, Wraparound Plan of Care, incident reports, mental health notes in a timely manner
  • Coordinate care and maintain communication with Child and Family Team members and community partners to support positive client outcomes
  • Coordinate the development of crisis and safety plans that reflect family-driven, proactive, and reactive strategies and provide crisis management as needed
  • Utilize crisis communication and de-escalation techniques, training in additional crisis intervention modalities may be required
  • Attend team meetings, required trainings, individual supervision, and group supervision

QUALIFICATIONS

REQUIRED:
  • Master's degree in Counseling/Social Work/Psychology
  • Registered with California BBS: ASW, AMFT, APCC, or full licensed
  • Bilingual Spanish skill set, pass bilingual proficiency assessment
  • Be part of a rotating emergency on-call system, scheduled ahead of time
  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies' insurance policy
  • Good organizational and writing skills
  • At least 21 years of age
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements

PREFERRED:
  • Experience implementing family-focused Wraparound services
  • Demonstrate cultural responsiveness and an understanding of implicit bias

SCHEDULE
  • Full-time; Monday - Friday, 9am - 5:30pm
  • On-call, three - four 24-hour shifts per month scheduled ahead of time
  • Hybrid, provide in-person services & complete documentation remotely

BENEFITS
  • Starting at $90,559 - $104,559 per year, commensurate rate with experience
    • Additional $4000 salary increase for BBS licensure
