Work Shifts8:00 A.M - 4:00 P.MDuties & ResponsibilitiesSUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Provides oversight of day-to-day operations within Quality Management, including performance of assigned staff including evaluating their performance against established performance measures; reviews and authorizes time off, overtime and other non-productive/productive time in accordance with time and leave procedures.
• Provides best practices to staff through one-on-one coaching, group interaction at case conferences, in-service training, and other team meetings.
• Monitors department activities to evaluate the productivity and quality of programs and processes to identify potential improvements and to ensure maximum performance.
• Manages and participates in departmental projects, workflow processes and review and/or creation of policies and procedures.
• Collaborates with staff and leadership to develop and implement systems that support operations and business goals within identified areas of responsibility.
• Provides supervision to open and closed record reviews utilizing chart review tools and EMR in accordance with departmental rules/regulations and Kings County policies and procedures.
• Provides supervision to review of concurrent and retrospective medical records, incident reports, and other documentation in order to identify actual or potential quality & risk concerns.
• Lead Hospital wide sepsis initiative meetings and monitors sepsis performance improvement activities.
• Ensures timely monthly upload of inpatient and outpatient file to Premier secure portal, including corrections to the errors book.
• Acts as liaison between the Quality Management department, local departments, disciplines and service-wide leadership to achieve organizational goals with respect to Performance Improvement and mission critical initiatives.
• Navigates various databases and systems both internally and externally in order to satisfy reporting requirements, including CMS, TJC, NYSDOH, Premier and Quintile Core Measure deadlines.
o These may include, but are not limited to, Premier, Midas, QHIP, NYSDOH, Quintile/Outcome, 3M 360 HDM, CMS Quality Net.
• Collect, organize, and analyze trends in data to inform Performance Improvement projects and ongoing problem-solving workgroups.
• Represent the Quality Management department in various organizational forums and meetings using the understanding of regulatory and accreditation requirements (DOH, CMS, TJC, etc.) for the purpose of guiding and informing ethical Performance Improvement institutional-wide projects.
• Trains and educates as well as demonstrates the use of and modeling of effective decision making, critical thinking and problem-solving techniques and tools.
• Participates in departmental and Hospital Wide Quality Councils.
Promotes a positive and pleasant attitude conducive to teamwork aiming to enhance and further the mission,
vision, and values of the facility.
• Provide training on Core Measures and performance improvement initiatives.
• Review Hospital Acquired Conditions and developed Quality Management Division Dashboard.
• Maintains confidentiality of all information as well as the release of any information.
• Performs other activities as assigned by the Chief Quality Officer and Director of Quality Management.
Minimum Qualifications* Qualification for Central Office.
# Qualification for Health Care Facility.
1. a. A Baccalaureate Degree in Public Administration, Public Health or Health Care Specialization; and,
b. Five* years or four# years of increasingly responsible experience in hospital administration, with particular emphasis on development and evaluation of health care delivery services; or,
2. a. A Masters Degree in Public Administration, Public Health or Health Care Specialization; and,
b. Four* years or three# years of increasingly responsible experience in hospital administration, with particular emphasis on development and evaluation of health care delivery services.
