Overview

As our Associate Director of Population Health, you’ll be responsible for developing and executing strategies to improve Value-Based Care (VBC) outcomes and operational effectiveness. You’ll also ensure the successful implementation of VBC initiatives aimed at enhancing care quality and optimizing performance in alignment with VBC models.

 

Selected Responsibilities:

  • Manage value-based care, population health, and care management team supporting both Avance Care and other participants of the Clinically Integrated Network (CIN).
  • Deliver and execute strategic plan for monitoring and improvement of value-based care measures across all practices in the CIN.
  • Collaborate with the Director of the CIN and Account Managers to communicate effectively with CIN participants.
  • Bring and execute a strategic plan to create, develop, and maintain a population health platform that addresses clinical and financial improvement across all relevant care categories including coordination with wellness, home, hospital, and specialty care.
  • Partners with Accountable Care Organizations (ACO) to monitor provider and practice engagement as well as management usage of ACO platforms, including customizations.
  • In conjunction with the CIN, create dashboards and feedback mechanisms for the practices to maximize ACO performance.
  • Oversee fulfillment of reporting requirements of commercial payers.
  • Other duties/administrative tasks as assigned.

 

The ideal candidate will bring at least a minimum of 5+ years of relevant experience in population health management with a track record of driving actionable insights for VBC success. Along with a bachelor’s degree required, a master’s degree, or other advanced degrees in health services research, health policy, mathematics/statistics, nursing, or other relevant fields is highly preferred.
