Pay Range: $95,000-$125,000

Position SummaryThe Advanced Practice Provider provides comprehensive, patient-centered primary care within the provider's education, training, licensure, and approved scope of practice. Responsibilities include assessment, diagnosis, treatment, preventive care, chronic disease management, patient education, and care coordination.
The provider practices in accordance with Indiana law, applicable professional standards, organizational policies, and any required collaborative practice agreement.
Responsibilities- Obtain and document comprehensive patient histories and perform physical examinations.
- Diagnose and manage acute and chronic medical conditions.
- Develop, implement, and evaluate individualized treatment plans.
- Order, perform, and interpret appropriate laboratory and diagnostic studies.
- Prescribe medications and treatments within applicable Indiana authority.
- Provide preventive care, screenings, immunizations, and chronic disease management.
- Educate patients regarding diagnoses, medications, treatment plans, and healthy behaviors.
- Coordinate referrals and collaborate with physicians, nurses, behavioral health professionals, and other members of the care team.
- Provide telehealth services when clinically appropriate.
- Maintain timely, accurate, and complete documentation in the electronic health record.
- Participate in quality-improvement, peer-review, clinical department, and staff meetings.
- Follow TCA Health policies, clinical guidelines, productivity expectations, and applicable regulatory requirements.
QualificationsNurse Practitioner- Current, unrestricted Indiana RN license or qualifying multistate compact RN license.
- Current Indiana APRN prescriptive-authority license, when required.
- Master's or doctoral degree from an accredited nurse practitioner program.
- Current national board certification in an appropriate primary-care specialty.
- Current Indiana Controlled Substances Registration and DEA registration, if prescribing controlled substances.
Physician Assistant- Current, unrestricted Indiana Physician Assistant license.
- Graduation from an accredited physician assistant program.
- Current NCCPA certification.
- Current Indiana Controlled Substances Registration and DEA registration, if prescribing controlled substances.
All Applicants- Current BLS certification.
- Eligible for credentialing and enrollment with Medicare, Medicaid, and other health plans.
- Primary-care or community-health experience preferred.
- Experience serving medically underserved populations strongly preferred.
