About the Opportunity:

  • The Outpatient Palliative CareAPPwill treat patients in their homes or facilities within the Moore County area.
  • The careteam includessevenPalliative CareAPPs andtwodedicated social workers.
  • Weekday position with protected administrative time.
  • Call Schedule 1:4 weeks with Hospice: M-F 5:00 pm- 8: 30 am, 24-hour coverage Saturday and Sunday.
