DescriptionThe After-Hours APP Clinical Manager provides clinical, operational, and quality oversight of the After-Hours Advanced Practice Provider Program. This role supervises After-Hours APPs and Team Leads, ensuring safe, high-quality care, effective treat-in-place interventions, reduced avoidable hospitalizations, regulatory compliance, and consistent execution of the Longevity Health Model of Care. The position serves as a liaison between clinical leadership, Medical Directors, telehealth operations, and facility partners to support after-hours services across evenings, nights, weekends, and holidays.
Key ResponsibilitiesClinical & Operational Leadership- Supervise and support After-Hours APPs.
- Oversee after-hours clinical operations, staffing, scheduling, and workflow management.
- Serve as an escalation resource for complex clinical and operational issues.
- Ensure effective communication and care transitions between after-hours and daytime teams.
- Collaborate with Medical Directors and clinical leadership on high-risk cases, utilization trends, and operational performance.
- Support recruitment, onboarding, retention, and development of after-hours staff.
Quality, Utilization & Compliance- Conduct audits of documentation, clinical decision-making, treat-in-place outcomes, and regulatory compliance.
- Review hospital transfers, readmissions, deaths, and significant clinical events to identify improvement opportunities.
- Monitor utilization metrics, hospitalization rates, transfer appropriateness, and member outcomes.
- Develop corrective action plans and performance improvement initiatives when needed.
- Ensure compliance with organizational policies, CMS requirements, HIPAA regulations, and Model of Care standards.
Performance Management & Staff Development- Conduct routine coaching, performance reviews, and one-on-one meetings.
- Monitor productivity, quality scores, documentation standards, and operational metrics.
- Support continuing education, competency development, and leadership growth.
- Address performance concerns in partnership with Human Resources and clinical leadership.
Communication & Collaboration- Partner with Medical Directors, MDCSs, Operational Leaders, Telehealth teams, and nursing facility partners.
- Communicate significant clinical, safety, quality, and operational concerns to leadership in a timely manner.
- Facilitate team meetings, education sessions, operational updates, and quality reviews.
- Promote a culture of accountability, collaboration, and continuous improvement.
- Perform other related duties as assigned.
RequirementsQualificationsEducation & Credentials- Master of Science in Nursing as a Nurse Practitioner
- Active unrestricted NP license and board certification required.
- DEA registration, active NPI, and Medicare participation eligibility required.
- APP licensure and credentialing may be required in states utilizing the After-Hours Program based on organizational needs.
Experience- Minimum of three years of APP clinical experience required.
- Leadership, supervisory, preceptor, or team lead experience preferred.
- Experience in long-term care, geriatrics, post-acute care, hospice, palliative care, ISNP, telehealth, or utilization management preferred.
- Strong knowledge of treat-in-place strategies, hospitalization reduction initiatives, and nursing facility operations.
- Computer proficiency with Microsoft Office, EMR platforms, telehealth technology, call management systems, and reporting tools.
