CORE JOB SUMMARY
The Advanced Practice Registered Nurse (APRN) delivers medical care to a wide variety of patients. The APRN also examines and treats patients independently and in autonomous collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. May prescribe medications and order diagnostic tests. Advises patients about continuing care.

  • Performs and documents complete physical examinations and comprehensive health histories.
  • Functions independently to perform age-appropriate history and physical for patients.
  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
  • Implements interventions to support the patient to regain or maintain physiologic stability.
  • Assists with the provision of care in accordance with facility, state, and federal regulations.
  • Monitors the effectiveness of interventions.
  • Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients.
  • Collaborates with multidisciplinary team members by making appropriate referrals.
  • Facilitates staff, patient and family decision making by providing educational tools.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.

Department Specific Functions

  • Provides comprehensive primary care services to adult patients in an ambulatory setting.
  • Functions as a core member of a Team-Based Care pod, collaborating with physicians, registered nurses, medical assistants, and administrative staff to deliver coordinated, patient-centered care.
  • Evaluates, diagnoses, treats, and manages acute and chronic medical conditions within scope of practice.
  • Conducts independent patient visits, including same-day appointments, acute care visits, chronic disease follow-up, preventive care visits, annual wellness visits, transitional care management, and post-hospital discharge follow-up.
  • Collaborates with physicians to independently manage appropriate patient visits, improving patient access, continuity of care, and provider capacity.
  • Orders, performs, and interprets laboratory, diagnostic, and imaging studies and develops evidence-based treatment plans.
  • Prescribes medications, including controlled substances, in accordance with state regulations, DEA requirements, and organizational policies.
  • Performs medication management, reconciliation, and patient education to support treatment adherence and clinical outcomes.
  • Assists with pre-operative evaluations and coordination of care across the continuum.
  • Reviews and addresses patient messages, medication refill requests, laboratory and diagnostic results, and other clinical tasks within established department standards.
  • Participates in daily huddles, care coordination activities, and workflow optimization initiatives to support efficient care delivery and patient access.
  • Documents patient encounters and clinical activities accurately and timely within the electronic medical record.
  • Maintains productivity, quality, patient experience, and access metrics established by the department.
  • DEA license required.
  • Participate in after-hours call coverage as assigned in accordance with departmental needs and established protocols.
  • Minimum three (3) years of experience as an Advanced Practice Registered Nurse in adult primary care required.
  • Performs other duties as assigned by the Chief Medical Director and/or Medical Director.
