WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a full-time Nurse Practitioner or Physician Assistant with Palliative Care experience for a position at Franciscan Hospice & Home Care in Hobart, IN to meet the growing demands for outpatient at home health Hospice and Palliative Care services. Provider will see patients in various venues of care such as personal residence, assisted living facilities, and skilled nursing facilities.

WHAT WE NEED:

Ideal candidate will have 2 to 5 years of Palliative, Home Health or Primary Care experience, with a focus on clinical excellence and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.

WHAT YOU CAN EXPECT:
  • Schedule hours: Monday - Friday, 8:00 a.m. - 5:00 p.m.; No weekends or holidays (schedule flexibility with high autonomy available)
  • Home based and community facility visits for pain and symptom management
  • Coverage area includes Lake, Porter & LaPorte Counties; Focus on northwest IN counties is available
  • Offices located in Hobart, IN and Lafayette, IN
  • Mileage reimbursement or fleet car provided
  • Average patient volume: 5 to 10 patients per day
  • Experience in Palliative Care, Home Health Care, or Primary Care preferred
  • Telehealth offered
