WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a full-time Nurse Practitioner or Physician Assistant with Palliative Care experience for an Inpatient and outpatient position to join our team at Franciscan Health Dyer and Franciscan Health Munster to meet the growing demands for Hospice and Palliative Care services.

WHAT WE NEED:

Ideal candidate will have past Palliative Care experience, with a focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.

WHAT YOU CAN EXPECT:
  • Schedule: Time split between clinic and inpatient rounding
  • Rotating weekend call for inpatient only: 1:4
  • Average outpatient volume: 12 patients per week
  • Average inpatient volume: 6+ patients per day
