Reliant Medical Group, part of the Optum family of businesses, is seeking a Advanced Practice Clinician (NP/PA) to join our Skilled Nursing Facility (SNF) team in Worcester , MA.Position Highlights & Primary Responsibilities:- Seeking a NP or PA with two years of working experience as an APC
- Our ideal candidate will have prior homecare experience.
- Provide in-home primary care to our most frail and at-risk elderly patients
- Have ownership of your patient panel
- Care for an average of six to eight patients daily
- Provide coverage for Metrowest communities

Required Qualifications:- National Board Certification
- Licensed by the Commonwealth of Massachusetts board of registration in Nursing
- Graduate of an Accredited Nurse Practitioner Program, Masters level preferred
- Maintains BLS certification, professional certification as appropriate
Preferred Qualifications:- Minimum two years of APC Experience
- Geriatric experience
- Homecare experience
- Fluency in EMR systems and technology
Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
