Responsibilities

Job Title: Advanced Practice Provider

Location: York, PA

Employment Type: Full time

What You'll Champion:
  • Schedule: 4x10-hour shifts (7:00 a.m. - 5:00 p.m.) offering work-life balance
  • Rotating inpatient/outpatient schedule (4 weeks outpatient / 2 weeks inpatient) for variety and skill development
  • Minimal call coverage with strong team support (approx. 2-3 calls/month; inpatient provider manages evenings/weekends)
  • Adult-only cardiology patients with a focus on heart failure management
  • Provide comprehensive care for heart failure patients including diagnosis, medication management, treatment planning, and patient education
  • Serve as a primary point of contact for patients with chronic cardiovascular conditions
  • Structured, extended orientation with hands-on mentorship alongside experienced Nurse Practitioners and Physician Assistants
  • Open to experienced APPs or new graduates with strong interest in cardiology or heart failure management
  • Cardiovascular experience and/or foundational knowledge in heart failure care strongly preferred
  • Collaborative, multidisciplinary cardiology team with a patient-centered care approach
