Scope of Work:

Advanced Practice Provider (NP/PA) or Physicians on Flourish Health Clinic

Key Responsibilities:

Flourish Health Clinical Service Delivery


1. Provide direct medical care across the mobile clinic service lines including:

  • Acute care (e.g., minor infections, musculoskeletal issues, wound care, etc.)

  • Primary Care Services (e.g., hypertension, diabetes, COPD)

  • Preventative care services (e.g., screenings, administration of immunizations)

  • Perform medical evaluations, diagnoses, and treatments according to mobile clinic protocols

  • Provide or oversee point-of-care testing (e.g., glucose, A1C, lipid panels, rapid HIV/HCV, COVID/influenza)

  • Dispense medications

  • Prescribe and monitor pharmacologic therapies, including non-opioid pain management and SUD-related therapies

  • Participate in overdose response and provide naloxone as clinically indicated

  • Collaborate in behavioral health and crisis intervention efforts, including initial assessments and referrals

  • Lead or co-lead interdisciplinary care discussions with staff and healthcare professionals on units and students

  • Assist in developing and reviewing clinical treatment protocols and patient care workflows

  • Ensure accurate documentation of all patient encounters and clinical services in the EMR

  • Participate in training and mentorship of student, residents, and fellows in clinical care

  • Assist in maintaining inventory and regulatory compliance for clinical and medication services

  • Support monitoring of clinical service metrics and patient outcomes

2. Harm Reduction & Substance Use Disorder (SUD) Best Practices

  • Deliver patient-centered care grounded in harm reduction and trauma-informed practices

  • Administer or supervise naloxone distribution, fentanyl test strip education, and safer use counseling

  • Support screening and referral for MAT (medication-assisted treatment) and behavioral health services

  • Serve as a clinical lead in implementing evidence-based harm reduction approaches in mobile workflows

  • Participate in training and mentorship of student and resident learners on stigma reduction and culturally responsive care

  • Provide clinical feedback and recommendations during program evaluation and CQI processes
