well as with patients, vendors, contractors, and related resources.

PRIMARY TASKS & DUTIES

  1. Functions independently to obtain comprehensive medical history from patient, and/or family, and/or previous medical records


  1. Prescribes medications according to therapeutic goals; explains prescription and treatment to patient and families


  1. Documents thoroughly in the EHR


  1. Teaches and counsels patients and families


  1. Assess, diagnosis, implements treatment, evaluates and monitors patients' health status for all age groups within the scope of practice.


  1. Performs complete physical examinations on patients of all age groups within the scope of practice


  1. Orders routine and diagnostic lab and radiologic studies according to standing orders


  1. Diagnose acute and chronic illnesses based on clinical findings, laboratory and radiologic reports and/or the results of other diagnostic procedures according to approved protocols


  1. Refers patients requiring hospitalization, or assessment and management of conditions, to supervising physicians or appropriate specialist. Follows patient's progress with physician


  1. Consults with physicians and other members of the health care team as necessary


  1. Performs other necessary duties as required by the community health center to meet the goal of providing primary health care services.


  1. Take call per the clinics on call schedule and GHC On-Call Procedure, within the scope of practice.


  1. Serve as relief coverage for all qualified positions, as needed


  1. Must be able to commute to other Genesis Healthcare, Inc. locations, as needed

POSITION REQUIREMENTS

Education
• Graduate of an accredited Nurse Practitioner Program or Physician Assistant Program
• Board Certification or Eligibility for Board Certification
• Has current license or ability to obtain licensure of practice in South Carolina
• Must be qualified in Basic Life Support techniques

Professional
• Minimum experience of 2 years in a Family Practice Mid-Level role, preferred but not required.
• Must have sufficient experience to carry out the duties of this position.

Physical/Environmental
• Visual/hearing ability sufficient to comprehend written/verbal communication.
• Ability to perform tasks involving physical activity, which may include lifting of supplies or equipment.
• Extensive bending, standing, walking or sitting may be required.
• Ability to interact with computer screens for up to six hours at a time.
• Ability to deal effectively with stress.
