Position Summary:

The Medical College of Wisconsin's Department of Pediatrics- Complex Care, Division of Complex Care and Palliative Care , is seeking an Advanced Practice Provider (APP) to provide care within the specialty clinic and inpatient. This position is located in Milwaukee at Children's Wisconsin. The APP works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching.

Position Highlights:

• Direct Care and Medical Care Coordination: Use Care coordination as a strategy to assist in implementing the plan of care for special needs patients in collaboration with the family, nurses and physician in the Comple Care Program, primary care, subspecialists and other providers. Examine and assess patients, prescribe medication and prescribe changes in treatment under the supervision of the Complex Care Program physicians. Synthesize and evaluation the patient/family responses to therapy; make independent judgments; offer input on discharge needs; evaluation effectiveness of discharge planning as a transition to home and adaptation to illness. Provide expert advice, which incorporates the broader aspects of patients/families response to illness. Seek opportunities for providing information and initiating consultation. Responsibilities include inpatient rounds, outpatient clinic visits, home visits occasionally, writing and updating comprehensive clinical summaries in compliance with program procedure, documentation, and billing procedures.
• Advocacy: Advocate for children with special health care needs and their families in healthcare, public policy, and with community organizations and in other forums.
• Education and Outreach: Present educational programs and conferences. Communicate content for formal and informal teaching and act as resource for patient/ families, staff and community. Participate in Comple Care research and quality improvement efforts.
• Evidence Based Practice: Apply current research findings in own practice. Introduce current research into clinical practice. In collaboration with the Complex Care team, develop and implement research proposals that fit with the program's mission and goals. Contribute to the body of knowledge of nursing and related disciplines.
• Program Development: Contribute to program development efforts, particularly clinical care improvement and activities in collaboration with the Complex Care Team. Collect patient-related data to comply with program requirements and needs. Participate in professional improvement activities including professional organization membership.
• We collaborate with all subspeciality. Our team dynamic is comprised of 4 Faculty, 11 APPs, 1 Psychologist, 11 RN Case Mangers, 10 Care Coordinators, 2 Social Works and 2 Administrative Assistants.
• Required to work on the Milwaukee Children's Wisconsin main campus
• This role would report to the Complex Care APP Manager.

Schedule:

• Full-time Position
• Expectations for coverage during core business hours and flexibility required as necessary to accommodate business needs
• 2-3 call nights per month and 1-2 holiday per year
• 11-12 weeks of inpatient services on MILWAUKEE CAMPUS per year which includes weekends and call
• Required: 1.0 clinical FTE
• Required: Salaried (Exempt)

Complex Care APPs pratice at the highest scope of pratice with an independent case load and flexible clinic schedule. Strong retention with APPs in complex care ranging between 2 to 15 years of experience. Strong collaboration with all subspecialties at Children's Wisconsin

Qualifications:

Preferred Experience:

Previous APP proficiency in complex professional and administrative skills usually acquired through 2-3 years of clinical experience preferred.

Minimum Education:

• Advanced Practice Registered Nurse: Master's Degree or higher in Nursing from a nationally accredited organization.
• Physician Assistant: Bachelor's Degree or higher in Physician Assistant Studies from an ARC-PA-accredited organization.
