Qualifications

  • Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of employment.
  • Possession of a Federal DEA License or eligibility.
  • Preferred experience or training in geriatrics.
  • Current CPR/BLS certification required.
  • Board Certification in relevant specialty is necessary.
  • Valid driver’s license with a clean driving record is mandatory.

Responsibilities

  • Manage chronic conditions for your own panel of geriatric patients.
  • Spend 30-45 minutes with each patient to address their needs comprehensively.
  • Prioritize patient care over volume of patients seen.
  • Collaborate with a team of over 250 providers to support high-risk Medicare patients.
  • Provide primary care services in line with established medical guidelines.

Overview

32hr/week (4-day) or 40hr/week (5-day) option, Mon-Fri, no weekends or evenings.  Chronic condition management of your own geriatric panel - spend 30-45min with your patients!

Responsibilities

The Advance Practice Providers (NP/PA) we seek are those who have directly related experience in geriatrics and primary care. The APPs who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications
  • Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
  • Federal DEA License/eligibility
  • Geriatric training/experience preferred
  • Active CPR/BLS Certification
  • Board Certification
  • Must maintain a valid driver’s license and good driving record
