Advanced Practice Nurse - Bridge Care Bergen County NJ Salary Range $125,000 to $135,000 a yearAdvanced Practice Nurse will be responsible for, but are not limited to:
- Conduct visits to patients in accordance with local, state and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team.
- Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
- Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts and documents.
- Assist in development of clinical practice guidelines/standards in support of quality care as needed.
- Ensure collaboration with patients, caregivers, medical directors and members of IDT.
Education: Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
Professional Experience: - Must have a Nurse Practitioner's license in the state of operations
- Must have 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field,
- Must have a valid driver's license and a reliable source of transportation
- Must be willing to travel
- Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
