Assistant Director of Nursing (ADON) - LVN
Vasona Creek Healthcare Center is seeking a dedicated, leadership-driven Assistant Director of Nursing (ADON) - LVN to join our clinical management team in Los Gatos, CA. This is a full-time, fully benefitted position offering a competitive compensation rate of $48.00 - $50.00 per hour based on experience.

Job Overview

As the LVN ADON, you will partner directly with our Director of Nursing (DON) to oversee clinical operations, lead nursing staff, and maintain high standards of patient-centered care. You will bridge the gap between administrative leadership and hands-on floor support, promoting a compliant, safe, and positive environment for our 148-bed skilled nursing and rehabilitation facility.

Key Responsibilities

  • Clinical Leadership: Assist the DON in overseeing daily nursing operations, clinical protocols, and state/federal regulatory compliance.
  • Staff Supervision: Manage, mentor, and schedule floor nurses (LVNs) and CNAs to optimize resident care and shift ratios.
  • Quality Assurance: Participate in quality assurance performance improvement (QAPI) audits and infection control monitoring.
  • Resident Advocacy: Oversee care plan implementations, coordinate with multidisciplinary therapy teams, and communicate facility updates directly to family members.
  • Incident Management: Investigate clinical incidents, document findings in PointClickCare (PCC), and implement corrective actions.
  • Floor Support: Step in to assist with clinical floor tasks, complex medication administration, or urgent medical assessments as required.


Qualifications & Requirements

  • Licensure: Active, unrestricted California Licensed Vocational Nurse (LVN) license in good standing.
  • Experience: Minimum of 2-3 years of clinical experience in a skilled nursing or long-term care setting.
  • Leadership Experience: Prior supervisor, Charge Nurse, or Unit Manager experience preferred.
  • Technical Skills: Proficiency with Electronic Health Records (EHR), with a preference for PointClickCare (PCC).
  • Regulatory Knowledge: Sound understanding of California Title 22 regulations and CMS guidelines.
  • Soft Skills: Exceptional conflict-resolution abilities, transparent communication skills, and a professional, supportive demeanor.
