Position Overview
We are seeking a highly skilled and compassionate Nurse Practitioner (PMHNP) to join our Assertive Community Treatment (ACT) Team. In this role, you will provide psychiatric evaluation, diagnosis, and medication management to individuals with severe and persistent mental illness in community settings. As a key member of a multidisciplinary team, you will collaborate closely with clinical staff to deliver integrated, recovery-oriented care.

Key Responsibilities
  • Conduct comprehensive psychiatric evaluations and ongoing assessments in community-based settings
  • Diagnose mental health conditions and develop individualized treatment plans
  • Prescribe, monitor, and adjust psychiatric medications in accordance with best practices and regulatory standards
  • Provide education to clients and families regarding diagnoses, medications, and treatment options
  • Collaborate with ACT team members to coordinate holistic care, including medical, behavioral, and social supports
  • Monitor client progress, response to treatment, and potential side effects
  • Participate in daily team meetings, case conferences, and care coordination activities
  • Provide crisis intervention and clinical support as needed
  • Maintain accurate and timely documentation in compliance with agency and regulatory requirements

Qualifications
  • Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) in NYS
  • Master's degree in Nursing required
  • Board certification as a Psychiatric Mental Health Nurse Practitioner
  • Minimum of 2 years of experience in psychiatric or behavioral health settings
  • Strong knowledge of psychiatric disorders, psychopharmacology, and integrated care practices
  • Excellent clinical judgment, communication, and interpersonal skills
  • Ability to work both independently and collaboratively within a multidisciplinary team

Preferred Qualifications
  • Experience working within an ACT team or community-based behavioral health program
  • Bilingual skills are a plus
  • Familiarity with electronic health records (EHR) systems
