Acclaim Health announces the availability of one (1) Full Time
Wound Advisor (RPN) (equivalent to 75 hours per pay period) to work in all Halton Clinics and Communities with a focus on the Mississauga/ Halton Communities & Clinics. The Individual is required to work Monday to Friday 8:30am - 4:00pm.
PRIMARY FUNCTIONS:- Provide nursing care for patients in all Halton Clinics, Communities and Mississauga.
- Utilizes Best Practice Strategies when developing, implementing, evaluating, and modifying the patient's plan of care
- Interacts and provides clinical insight with other health care professionals within the patient's circle of care
- Meets the psychosocial needs of patients with acute or chronic wound(s)
- Provides health teaching for patients and their families. Encourages and facilitates independent management of wound care
- Assesses, treats and makes treatment recommendations for persons with acute/chronic wounds
- Acts as a clinical resource for patients and nurses.
- Meets the requirements set out by Ontario Health at Home regarding assessment and follow-up with patients and adhering to the documentation requirements specific to the Wound Advisor
- Participates in multi-disciplinary care conferences as required
Pay Per Visit Rate: Range: $37.95 per visit - $41.72 per visit
POSITION REQUIREMENTS :- Minimum 1 to 2 years clinical practice as a nurse in a community setting preferred
- Previous wound care experience preferred
- Current CNO certificate of competence
- Current CPR certificate
- Excellent interpersonal, communication and teaching skills
- Demonstrates initiative and commitment to professional development and skill enhancement
- Wound Advisor Certificate (IIWCC)/Designation or equivalent
