A bit about this role:

We are seeking a Physician, Transitions of Care to provide exceptional patient care to recently discharged patients and advance our innovative clinical model focused on preventing hospital readmissions.

This position represents an exceptional opportunity for an enthusiastic physician leader to drive clinical excellence of Devoted Medical's Transitions of Care (TOC) Program. This is an ideal role for an experienced Hospitalist, Post-Acute Physician, Virtual Care Physician, or Value-Based Primary Care Physician with a background in Geriatrics, Palliative Care, or Complex Care Management looking to lead virtual care delivery and readmission reduction strategies.

The TOC program provides virtual care in the 30-day window after a patient is discharged from the acute setting (hospital or skilled nursing facility), serving patients across multiple states with the goal of preventing readmissions through exceptional, comprehensive wrap-around care. The program leverages cutting-edge technology and artificial intelligence (AI) to provide the best care possible.

The Physician will deliver high-quality clinical care to patients at high risk for readmission as part of a collaborative interprofessional team (IDT) of healthcare professionals, including nurse practitioners, physicians assistants, registered nurses, medical assistants, and clinical coordinators. This role combines direct patient care (approximately 70% of time) with team development, coaching for clinical excellence within an IDT, leading clinical problem-solving, and facilitating rounds (approximately 30% of time focused on clinical supervision, escalations, and clinical model improvement). Leadership in this role is focused on clinical supervision rather than direct administrative management.

You will be supported by advanced AI-enabled technology and comprehensive Devoted Medical programs - including virtual urgent care, specialty care such as palliative care, complex care management, and clinical pharmacy services. This role is designed for a physician with an entrepreneurial mindset who can leverage their deep clinical background to spearhead intricate quality improvement projects, cultivate a standard of clinical excellence within a distributed team, and champion the implementation of evolving AI technologies. In this capacity, the physician acts as the ultimate point for clinical escalations and manages oversight via collaborative practice agreements in various states.

Your Responsibilities and Impact Will Include:

Clinical Leadership & Patient Care (~70% of time):
  • Clinical Care for Transitions of Care: Deliver high-quality direct virtual patient care to patients recently discharged from the acute setting who are at high risk for readmission, focusing on complex transitions of care cases.
  • Clinical Escalations: Serve as the final escalation point for clinical questions and complex patient management decisions within your assigned interdisciplinary care team.
  • Clinical Outcomes Oversight: Oversee a narrow set of quality and clinical outcome metrics that are critical drivers of improved readmission rates.


Team Management & Clinical Supervision (~30% of time):
  • Clinical Supervision & Collaborative Practice: Provide supervision, teaching, and coaching to a team of Nurse Practitioners through collaborative practice agreements across multiple states, fostering a collaborative and supportive clinical environment.
  • Lead Interdisciplinary Rounds: Conduct weekly interdisciplinary rounds (3 hours/week) and readmission conferences to review high-risk patients, complex cases, and conduct deep-dive root cause analyses.
  • Interdisciplinary Collaboration: Provide clinical oversight and coaching for professionals across multiple disciplines. Collaborate closely with operations leads and senior managers for nursing, medical assistants, and clinical coordinators.
  • Foster an Open Clinical Culture: Champion a patient-centered and open clinical culture, fostering trust, transparency, honesty, and empowerment within a distributed team across geographic locations.


Technology, Innovation & Quality Improvement:
  • Clinical Innovation & Root Cause Analysis: Identify opportunities to improve the model of care through root cause analyses, identifying patterns of readmissions, developing strategies to prevent hospital readmissions, and driving quality improvement initiatives.
  • Technology & AI Enablement: Utilize and provide feedback on Devoted's proprietary electronic health information system (Orinoco) and contribute to the development, training, and application of AI-enabled clinical tools to support care within the TOC service.
  • Change Management & Innovation: Thrive in an entrepreneurial, high-growth environment with a high capacity for change management and agility. Drive innovation and process improvements to enhance the delivery of transitions of care services.
  • Advocate for Care Equity: Advocate for healthcare equity and accessibility within the organization and community, particularly for vulnerable patients at risk for readmission.


Required Skills and Experience:
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
  • Board certification in Internal Medicine, Family Medicine, or related specialty.
  • Minimum 3 years of post-residency clinical experience
  • Strong background in inpatient care and deep understanding of the transitions of care period.
  • Experience in the management of chronic medical diseases including heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections.
  • Experience leading interdisciplinary rounds and conducting deep-dive root cause analyses.
  • Direct experience supervising Nurse Practitioners or other healthcare professionals.
  • Proven ability to manage change, translate vision and strategy to front-line teams, and drive innovation in a dynamic healthcare environment.
  • Strong analytical and problem-solving abilities with experience using data to drive clinical and operational improvements.
  • Collaborative mindset with ability to work effectively with interdisciplinary teams and business partners


Desired Skills and Experience:
  • Experience in hospital medicine, post-acute care, primary care, or geriatrics
  • Previous experience as a medical director or in a similar physician leadership role within value-based care or Medicare Advantage organizations.
  • Experience delivering care in a remote/virtual environment or with telehealth platforms.
  • Background in quality improvement methodologies and leading clinical initiatives.
  • Experience working in high-growth healthcare organizations
  • Experience serving diverse patient populations, with a focus on delivering care that aligns with our mission.

Licensure and Certification:
  • Active medical license in good standing, with the ability and willingness to obtain licensure in additional states as the program expands (with support provided by Devoted).

Salary Range: $280,000 - $338,000 / year plus eligibility for discretionary performance-based bonus paid out annually (Target bonus is 10%, payout based on terms of the bonus plan).
