- 1st shift, Monday - Friday
- Remote role, with the ability to be onsite for meetings w/ your stakeholders. The expectation would be approximately once a month depending on needs.
Pay Range:
$38.20 - $57.30
You will lead the implementation of quality improvement initiatives for assigned ambulatory primary care practices/clinics. Apply project management, quality, and safety improvement frameworks, methodologies, and tools, evidence-based and/or best practice, and statistical theory and tools for data analysis. Guide achievement of Advocates strategic imperative of top decile performance in quality, safety, service, and cost-effective patient care while demonstrating compliance with regulatory agencies.
Major Responsibilities:
- Expertly leads performance improvement efforts andcoachesothers in the development of performance improvement capabilities with emphasis on process redesign and measurement; provides direction to assigned departments and customers within the organization in the planning of a systematic, organization-wide approach to continuous quality improvement.Applies a professional and calmstyle in challenging circumstances.
- Plans and leads moderately complex interdisciplinary projects, ensuring on-time delivery with high quality and coordination of resources. Collaboratively coordinates,facilitates, implements, andparticipatesin quality improvementand valueinitiativessuch asoptimizingutilizationin assignedambulatorygeographical areas. Participates in multidisciplinary quality improvementandvalue-basedcareteams and projects to support the accomplishment of quality improvement initiatives, value contract execution,and accreditation goals.
- AppliesAdvocate92s quality and high reliability improvement frameworks, methodologies, and tools, evidence-based practice, and data analysis to achieveoptimaloutcomes.Usesstatistical techniques and tools to analyze and display clinical data.Assistsin the development and enhancement of tools foraccuratedata collection and quality improvement initiatives. Reviews and analyzes quality data for accuracy, patterns, and trends. Participates in the development of improved Electronic Health Record tools to support Quality improvement initiatives in collaboration with key functional departments and leadership.Conductschart reviews, audits, surveys,and usesother data collection methodologies asindicated.
- Usessophisticated analytical thought to guide problem solving andidentifyinnovative solutions. Utilizes facilitation skills, dataanalysisand statistical process control to effect improvement in quality, value,and clinical outcomes. Maintains awareness of emerging conflicts and potential risk situations and uses effective techniques toameliorateorescalates toappropriate leadership for resolution.
- Leads department and performance improvement meetings, analyzes trends of performance metrics/business unit level dashboards,identifiesopportunities for improvement, develops andmaintainsmanagement action plans, measures success of these action plans and creates the records and follow-up metrics for each meeting and activity; reassesses approach to maximize success. Meets individually with clinicians and their teams toassistwith problem-solving, recommend andfacilitateimprovement strategies.
- Participates in the development of content for communication tailored to diverse audiences. Promotes cross-departmental communication, collaboration, and performance improvement.Ensuresall key stakeholders receive consistent communication across departments and sites. Communicates findings and analysis of record reviews, audits, surveys, and other data to process owners and stakeholders.
- Researchesandrecommendsbest practice strategies to improve processes and quality outcomes and goals. Serves as a resource to leadership and clinicians foridentifyingand developing innovative programs/services to enhance quality, safety, and efficiency of operational workflows.
- Participates in the development and maintenance of educationalmaterial. Provides training/educational programs and in-services to a wide range of audience, including but not limited to largegroupphysician meetings, office staff meetings, and on-site practice meetings, which can include clinical and non-clinical members. Serves as a resource and mentor to colleagues with less experience.
Licensure, Registration, and/or Certification Required:
- Certificate* in health care quality, patientsafetyor project management/Lean preferred and isrequiredwithin 2 years of employment.
- *Or equivalent internal program
Education Required:
- 020 Bachelor92s degree or equivalentyears of experience
Experience Required:
- Requires 3years' experiencecumulatively in health care, clinical quality improvement, projectmanagement,ora directly relatedfields.
Knowledge, Skills & Abilities Required:
- Knowledge of quality improvement methodologies, tools, and measures.
- Independent effective resolution of complex problems.

- Ability to travel to different practices within the market area
Preferred:
- Experience using Epic or other EMR system
- Ability to shift between priorities
- Direct experience within ambulatory in a hospital or medical practice.
- Strong process improvement experience

- Must have the ability to clearly communicate verbally, in person, and in writing with peers, leadership across varied departments and health care teams.
