Job Description
Job Title: Registered Nurse Case Manager
Profession: RN

Hours per Shift: 8 hours
Experience: Minimum one year of experience as a clinical nurse in a hospital required, and nine months experience in home care preferred.
License: Valid RN License (New York)
Certifications: BLS required
Must-Have:
- Certified Home Health Care experience
- EPIC or Netsmart experience
- Telemetry experience
- OASIS experience
Description:
The Registered Nurse Case Manager is responsible for developing and revising plans of treatment and discharge in conjunction with patients, families, and healthcare teams.
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharge, and significant change in condition as per agency-defined time frames.
Processes orders and notifies physicians of patient needs and changes in condition.
Creates an environment that promotes respect for patients' privacy, confidentiality, and property.
Initiates appropriate preventive and rehabilitative strategies.
Provides services requiring specialized nursing skills.
Educates patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies.
Supervises Licensed Practical Nurses to ensure patient health, safety, and compliance with care plans.
Directs, coordinates, evaluates, and supervises the quality of patient care services.
Interacts positively with patients, physicians, referral sources, and others.
Supports collaborative, customer-focused planning and case management to meet individual health needs and promote quality outcomes.
Assures compliance with state and federal regulations and timely completion of clinical records.
Maintains current knowledge of available community-based services.
Demonstrates flexibility and willingness to visit patients based on need.
Ensures resolution of problems at the point of contact.
Attends patient care conferences and participates in team discussions regarding assigned patients.
Participates in achieving operational and financial objectives of the agency.
Monitors results, progress, and quality indicators, including customer satisfaction.
Understands the impact of the RN function on patients, providers, readmissions, productivity, and expenses.
Pursues efforts to reduce avoidable costs and errors.
Responds to findings from quality management reviews and ensures resolution of identified issues.
Contributes to an environment of professional growth, learning, and respect for all staff.
Promotes recognition and respect for workforce diversity.
Participates in educational training and self-development as needed.
Demonstrates commitment to professional growth and competency.
Performs additional duties as requested.
