  • JOB DESCRIPTION DETAILS
    • Job Summary:
      • Collaborates with patients, families, advocates, healthcare providers and community resource representatives to promote health and increase community support to at-risk populations presenting to the health care system.
      • Performs utilization review and discharge planning to continue the patient through the continuum of care.
    • Education:
      • Position requires a BSN or an RN and a bachelor's degree in a related field, masters preferred.
    • Registration/Certification/Licensure:
      • RN - State of Maryland
    • Experience:
      • Position requires 3-5 years experience performing direct nursing care and 2 years case management experience in an acute care setting.
    • Other Requirements:
      • Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements.
      • Effectively communicates with all patient populations and staff by implementing organization-wide communication techniques as a daily practice.
      • Effectively advocates for quality patient safety and satisfaction as a daily practice.
      • Demonstrated proficiency of computer skills necessary to effectively complete position requirements.
      • Previous experience and demonstrated professionalism in effectively communicating with customers and in promoting a positive service atmosphere.
      • Ability to work independently in a multi-task environment producing quality work.
