Job Summary

Provides clinical and distributive pharmacy services to oncology patients. Incorporates educational, clinical, and technical pharmacy functions to prepare medications, assess and monitor medication therapy. Assists facility staff with medication-related issues and provides drug information to nursing and medical staff as well as developing oncology power plans and assisting with various oncology projects

Responsibilities
• Provides comprehensive pharmaceutical care to patients to ensure safe, efficacious and cost-effective use of oncologic and supportive care medications.
• Demonstrates independent decision-making, clinical judgement and in-dept knowledge of oncology therapeutics.
• Develops and maintains clinical practice, collaborating with medical and nursing personnel in pursuing optimal drug therapy for patients with cancer.
• Provides education and medication-related counseling to patients, families and caregivers about the drug therapy provided.
• Assists in the optimal selection of pharmacologic agents and design of a regimen to maximize therapeutic benefits and minimize toxicity.
• Support protocol standardization and Power Plan consistency across all Mon Health and Vandalia North oncology locations.
• Pharmacy support in clinical research.
• Recommends, verbally and in writing, adjustments or modifications to the drug regimen according to clinical and toxicity endpoint's, professional guidelines and evidence-based literature.
• Provides drug and clinical information to other healthcare professionals related to new/emerging technology, cancer treatments, and challenges encountered in the care of patients with cancer.
• Retrieves and evaluates relevant oncology literature.
• Participates in medication use evolution, guideline and policy development, prepares documents for P&T evaluation/consideration (formulator, etc.) and other projects as assigned.
• Participates in chemotherapy safety standard compliance.
• Participates in quality metric documentation required for CoC site visit readiness.
• Participates in cancer committee, including P&T Committee, Tumor board attendance, Medication Safety, and other relevant to the care of cancer patients.
• Follow procedures and define for the handling of high cost and off-label medication orders (site of care restrictions, retrievals of supportive literature, etc.).
• Integrates new information with existing information to establish recommendations for clinical use.
• Develops/maintains operation protocols and procedures in accordance with regulation and professional guidelines (ASCO, ONS, HOPA, USP, DNV, OHFLAC, WVBOP), and MHMC policy, ect.
• Responsible for the procurement, receipt, storage, compounding, dispending and labeling of medications.
• Maintains departmental, institutional, and licensure-required education. • Documents activate as required by regulatory/accreditation agencies, professional guidelines and MHMC policy.
• Demonstrates positive customer service and co-worker relations.

• Buy-and-bill workflow oversight across infusion sites.
• Travel to other infusion sites within the system as needed.
• Develops and communicates, verbally and in writing, specific pharmacotherapeutic plans for the supportive care of individual patients.
• Maintains a readily retrievable and ongoing record of each patient's chemotherapy regimen, dosages, and pertinent clinical and laboratory data.

Knowledge, Skills & Abilities

Experience
• PGY2 specialized in Oncology or a minimum of 7 years relevant oncology experience
• Direct experience supporting bispecific T-cell engager (BiTE) therapy, including step-up dosing logistics, REMS and monitoring requirements, CRS and ICANS recognition and management, tocilizumab availability and on-hand stocking requirements, and inpatient observation-to-outpatient transition workflows. CAR-T experience is appreciated but is not required.
• ONS/HOPA chemotherapy competency familiarity
• CPOE or Power Plan build experience

Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs with a maximum lifting of 25 lbs.

Credentials
• Board Cert Oncology Pharmacist Board Certified Oncology Pharmacist certificate within 24 months of hire.
