Overview:

Overview

The Supervisor, Care Coordination assumes responsibility, accountability and leadership for the daily operations including coordination of work, quality, and service. First line supervisor in the Department of Managed Care for assigned site/function. Facilitates the work of assigned team members. Provides a leadership role in ongoing case manager competency assessment, needs identification and educational offerings. Provides educational services and clinical oversight to the Managed Care Coordination team. Participates in the work activities of assigned teams and provides care coordination services as needed.

Requirements:

Education

• Master's Level Degree in Counseling, Social Work or Psychology (Required)

Certification/Licensure

• Licensed Clinical Social Worker License (Required)
• Licensed Marriage and Family Therapist (LMFT) (Preferred)
• Licensed Professional Counselor (LPC) (Preferred)

Experience

• 3 years Case Management experience (Required)
• 5 years Acute Care experience (Required)
• 1-year experience working with vulnerable populations (Required)
• Prior supervisory experience (Preferred)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $97,843.20 - $152,006.40 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
