The Nurse Manager provides leadership and oversight for the Street Medicine Program, including community-based nursing, healthcare navigation, and care coordination for individuals experiencing homelessness. Utilizing trauma-informed care, harm reduction, and motivational interviewing, the Nurse Manager assesses health needs, provides basic nursing care and health education, connects clients with healthcare and community resources, develops individualized care plans, and coordinates ongoing follow-up care. The Nurse Manager collaborates closely with street outreach teams, case managers, healthcare providers, housing partners, and community organizations to improve health outcomes, increase access to care, and support housing stability.
Key Responsibilities- Manage the Street Medicine Program and provide supervisory leadership to three full-time case managers and student interns.
- Provide direct supervision to three employees and indirect supervision and oversight to university and hospital students, interns, and residents participating in Street Medicine activities.
- Carry out supervisory responsibilities in accordance with organizational policies, applicable laws, and professional standards.
- Provide field-based nursing assessments, health screenings, vital signs, medication reconciliation, basic nursing interventions, minor wound care, and health education within the RN scope of practice.
- Assess physical health, behavioral health, and substance use needs and connect clients to appropriate primary care, specialty care, behavioral health, substance use treatment, Medication-Assisted Treatment (MAT), pharmacy, and emergency services.
- Develop individualized care plans and support medication adherence, treatment recommendations, preventive care, and chronic health management.
- Coordinate medical appointments, referrals, medications, transportation, hospital follow-up, and ongoing healthcare needs.
- Utilize trauma-informed care, motivational interviewing, and harm-reduction practices to support client-centered health and wellness goals.
- Provide education on self-care, medications, chronic health conditions, reproductive health, HIV/AIDS, mental health, substance use, harm reduction, and other health-related needs.
- Collaborate with street outreach teams, case managers, healthcare providers, housing partners, and community organizations to address barriers to healthcare access and housing stability, including housing referrals, applications, documentation, and follow-up.
- Identify urgent and emergent health concerns and facilitate appropriate medical evaluation, treatment, or emergency care.
- Maintain accurate, timely, and compliant documentation in HMIS, the Electronic Health Record (EHR), and other required systems.
- Maintain the safety, security, and readiness of the mobile unit, including medical supplies, equipment, PPE, wound-care materials, and client care materials.
- Participate in and support community health events, including health fairs and health education activities.
- Participate in quality improvement activities, stakeholder meetings, and community partnerships to strengthen service delivery and advocate for resources supporting individuals experiencing homelessness.
- Ensure compliance with professional nursing standards, clinical protocols, infection-control and safety procedures, organizational policies, applicable laws and regulations, and ethical requirements. Protect client privacy and confidentiality in accordance with HIPAA and other applicable privacy regulations.
- Support and coordinate educational experiences for students, interns, residents, and other healthcare learners.
- Perform other related duties as assigned.
Qualifications- Current Pennsylvania RN license required; valid driver's license required.
- Minimum two years of clinical experience, preferably in community health, street medicine, public health, or working with vulnerable populations.
- Experience working with individuals experiencing homelessness, substance use disorders, mental illness, and complex medical needs.
- Ability to perform basic health screenings and point-of-care testing and transport medical supplies to community sites.
- Knowledge of and commitment to harm reduction, trauma-informed care, and public health.
- Ability to collaborate effectively with a diverse, multidisciplinary team.
- Strong communication, leadership, crisis intervention, problem-solving, and organizational skills.
- Commitment to ethical nursing practice, HIPAA, confidentiality, and applicable healthcare regulations.
- Proficiency with electronic health records and database systems; HMIS experience preferred.
- Bilingual English/Spanish preferred.
