The Start of Care Nurse plays a critical role in initiating home health services for new patients. You will perform comprehensive assessments and develop individualized care plans to ensure continuity and quality of care.Pay Rates:- Initial Start of Care: $150 per visit
- Follow-Up Visit: $90 per visit
- 90-Day Post-Operation Visit: $60 per visit
- Aide Evaluation Visit: $40 per visit
Responsibilities:- Conduct initial patient assessments and complete all required documentation.
- Develop and implement individualized care plans in compliance with regulatory standards.
- Educate patients and families on care procedures, medications, and health management.
- Coordinate with interdisciplinary teams to ensure seamless transitions and ongoing care.
- Maintain accurate records and ensure compliance with agency and state guidelines.
Qualifications:- Current and valid RN license in New York State.
- Strong knowledge of OASIS documentation and regulatory requirements.
- Excellent organizational and communication skills.
- Ability to travel to patient homes within assigned territory.
