Overview

Presbyterian Homes & Servicesis seeking a Staff Pharmacist to join its Radius Living Rx team.

The Staff Pharmacist reviews prescriptions, oversees the dispensing of medications, and works closely with other health care professionals to ensure safe and effective treatment for patients at Radiuss long term care facility partners. The Staff Pharmacist reports to the Pharmacist in Charge.

Radius Living Rx is a long-term care pharmacy serving residents in partner communities across Minnesota and Wisconsin. Our pharmacists, technicians, and support staff work closely with facility care teams and prescribers, so the work stays close to the people its for. Were building a team that wants pharmacy practice grounded in that relationship: careful medication management, direct communication with facility staff, and a genuine stake in how residents are doing.

Responsibilities
  • Accurately prepare medications for final dispensing to the patient, including filling, order entry, and packaging.
  • Verify the legality and pharmaceutical feasibility of dispensing a drug based on patient allergies, incompatibilities with current medications, frequency of refills, and related factors.
  • Interpret prescription orders, exercising professional judgment in determining whether to dispense a particular prescription, taking patient safety and legal limitations into account.
  • Consult with prescribers, nurses, and patients about drug interactions, adverse effects, medication storage, and other requested or pertinent prescription drug information.
  • Verify correct medication, directions for use, and other prescription information in accordance with Radius policy and state and federal regulation prior to dispensing.
  • Provide guidance to pharmacy technicians, pharmacist interns, and other pharmacy support personnel performing pharmacy related duties.
  • Compound nonsterile prescriptions in accordance with USP <795> and state and federal regulation as prescribed, calculating, weighing, measuring, and mixing ingredients.
  • Adhere to all federal and state Board of Pharmacy regulations, and meet or exceed daily workflow expectations.
  • Demonstrate good stewardship of available resources. Assist in the care and maintenance of departmental equipment, supplies, work areas, and facilities; obtain supplies and equipment as needed.
  • Promote continuous self-development and growth. Maintain current knowledge of community resources and organization and industry policies and practices. Use discretion and independent judgment to compare and evaluate possible courses of action and decision making.
  • Support the integration of Christian Culture through communication and education systems.
  • Other areas as identified to support aligned goal setting and achievement as driven by the Pharmacist in Charge.
Qualifications

  • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
  • Demonstrated compatibility with PHS mission and operating philosophies.
Pay RangeThe estimated starting pay range for this role is $130,000 - $150,000 per year. This good faith estimate is based upon PHSs typical starting pay range for this role. Actual pay range depends upon many factors including each individual employees qualifications and market conditions and is subject to change. Benefits

Full-time benefits-eligible employment begins at a minimum of 30 hours per week:
