Job Title: Speech Pathologist
Department: Therapies- Home Care
Location: Based out of 330 Monroe Avenue, supporting the West team area-includes part of Greece, Spencerport, Gates, Chili and SW city.
Hours Per Week: 40 hours per week
Schedule: 8:00am-5:00pm Monday through Friday

RESPONSIBILITIES - Patient Evaluation & Development. Meet the patient's goals and needs by providing quality, evidence based care; assess, develop and implement an individualized treatment plan
- Patient Care. Help patient accomplish treatment plan and accept therapeutic devices/recommendations; instruct, encourage and assist patients in performing daily-living activities and in using assistive and supportive devices
- Leadership. Develop the speech-language therapy staff; provide information, develop and conduct in-service training programs
EDUCATION:MS: Speech Pathology (Required)
LICENSES / CERTIFICATIONS: CCC-SLP - Certificate of Clinical Competence in Speech-Language Pathology - American Speech-Language-Hearing Association (ASLHA)American Speech-Language-Hearing Association (ASLHA), SLP - Speech-Language Pathologist - New York State Education Department (NYSED)New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
