What You'll Do:
- Perform patient evaluations and develop individualized treatment plans
- Provide evidence-based speech therapy interventions
- Travel to nearby assisted living and memory care communities to provide on-site therapy services
- Collaborate with families, caregivers, and interdisciplinary teams
- Maintain accurate documentation and assist with discharge planning
- Participate in community outreach and wellness programs
What We're Looking For:
- Licensed Speech-Language Pathologist (SLP) in Arizona
- Strong clinical reasoning and communication skills
- All experience levels welcome-new grads encouraged
- Interest in outpatient rehab and working within assisted living, independent living, and memory care settings
- Willingness to travel locally between clinic and community sites
