Job DescriptionAs a valued member of the interdisciplinary care team, the Social Worker is responsible for assessing, evaluating, and addressing the psychosocial needs of pediatric patients and their families. Using professional judgment, clinical expertise, and a collaborative approach, the Social Worker develops and implements care plans that promote patient safety, well-being, and positive outcomes.
The Social Worker champions Patient- and Family-Centered Care principles by advocating for patients, providing emotional support, and connecting families with appropriate community and healthcare resources. Responsibilities include delivering direct social work services, coordinating care, educating patients, families, and healthcare team members, and facilitating access to support services.
This role requires proactive assessment and anticipatory planning to identify and address potential patient and family needs before challenges arise. The Social Worker works closely with physicians, nurses, and other healthcare professionals to ensure resources are utilized effectively and efficiently while supporting quality care delivery.
The Social Worker is expected to maintain professional competencies, participate in ongoing education and development activities, and engage with community organizations and professional associations to enhance patient care and support community health initiatives.
Schedule:- Full-time | 80 hours every two weeks | St. Cloud
- Day shift | 8-hour shifts | Monday - Friday 8am-4:30pm
Pay and Benefits:- Starting pay begins at $30.65 per hour; exact wage determined by years of related experience and education level
- Pay range: $30.65 - $50.72 per hour

Qualifications:- Bachelor's degree or higher in Social Work, or bachelor's degree in related field; Master's preferred
- Experience with individuals who have special health care needs
- 2 years or more of clinical experience as a Social Worker
- Licensed Social Worker (LSW) Current and valid license to practice in the State of MN required
- Epic experience is a plus
- Critical thinking and organizational skills
- Ability to work collaboratively with multidisciplinary team
- Excellent oral and written communication
To be eligible for the sign-on bonus you must be an external candidate who is hired into an eligible full-time or part-time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.
To be eligible for the relocation bonus you must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. Must currently reside more than 50 miles away and relocate to within 30 miles of primary work location within 120 days.
