As a Social Worker MSW, a typical day might include the following:- Providing vital social work interventions, including supportive counseling and psychotherapeutic approaches, to address acute psychosocial distress.
- Conducting comprehensive patient assessments and maintaining detailed clinical documentation to support the care plan.
- Executing effective, strategic discharge planning to seamlessly optimize inpatient capacity and maintain smooth patient flow.
- Collaborating closely with an interdisciplinary team, including physicians and clinical nurses, during routine care rounds.
- Evaluating and addressing health-related social needs by actively incorporating social determinants of health into standard care planning.
- Overseeing seamless transitions of care and detailed discharge planning to ensure patient safety and continuity of care.
- Participating in targeted department initiatives to improve key performance indicators, such as length of stay and throughput, in alignment with RWJBH System Goals.

To be considered for this opportunity, you must have:- Master's degree in Social Work (MSW) from an accredited program required.
- Current Licensed Social Worker (LSW) OR Licensed Clinical Social Worker (LCSW) licensure required.
- Two (2) or more years of professional experience in a social work setting required.
- Ability to obtain Emergency Action Plan Coordinator (EARC) certification within six (6) months of hire.
- Prior professional experience within home health or post-acute services is preferred.
- Full Time, Day Shift: Monday through Friday, 8:00 AM - 4:30 PM.
- Weekend rotation is required approximately once every 6 weeks.
- Mandatory coverage of two (2) holidays per year.
