Pay Range:$72,800 - $95,000 per year (based on experience and qualifications)

We are seeking a compassionate and dedicated Social Worker (MSW) who is committed to uplifting the well-being of our residents. In this role, you will help guide kupuna and their families through challenges, transitions, and opportunities for healing. Your expertise will shape care plans, strengthen family connections, and ensure every resident receives the dignity and support they deserve.

Our desired candidate will have:
  • Master of Social Work (MSW) from an accredited institution
  • At least 2 years of experience in healthcare, LTC/SNF, geriatrics, behavioral health, or case management
  • Strong understanding of social work theories, interventions, and techniques
  • A heart for serving individuals from diverse cultural and socioeconomic backgrounds

What You'll Do:
  • Conduct meaningful assessments to understand residents' needs, strengths, and goals
  • Develop individualized treatment plans that honor each resident's story
  • Provide counseling, emotional support, and guidance through life's challenges
  • Connect residents with vital community resources, including mental health services, benefits, housing assistance, and support programs
  • Collaborate with nursing, therapy, and interdisciplinary teams to ensure holistic, person-centered care
  • Maintain accurate, confidential records that reflect the highest standards of professionalism
  • Participate in care conferences and team meetings to advocate for residents' needs
  • Stay informed about community programs to better serve residents and families
