Pay Range: $78,877.50 - $96,720.00 per year

Job Overview:

RWJBH is seeking a Clinical Social Worker to join our Advanced Heart Department! This position is for direct patient-centered care and advocacy within an interdisciplinary healthcare team. The role focuses on conducting psycho-social assessments, providing age-specific counseling, and managing complex discharge plans. You will connect families with community resources while maintaining a safe environment and ensuring seamless post-acute continuity of care.

Qualifications:

Required:
  • Licensed Social Worker with Masters Degree in Social Work from an accredited school of social work required
  • Experience working in a health care setting or human services organization required


Preferred:
  • LCSW preferred
  • Two or more year's experience as a transplant, renal or medial social worker is highly preferred


Scheduling Requirements

  • Full Time 35 hours per week
  • Day Shift


Essential Functions:

  • Conduct a comprehensive psychosocial assessment/evaluations, specific to determining an individual s capacity to receive an organ and/or VAD while identifying all issues for pre transplant resolution
  • Document all patient evaluations and interactions in a meaningful, timely and concise manager in the appropriate location in the patient record
  • Communicate all findings to the multidisciplinary team
  • Facilitate therapeutic support groups or educational sessions to provide meaningful assistance to target audience,
  • Identify and employee interventions or communication to ensure the best possible transplant/VAD outcomes by reducing the impact of known psychosocial risk factors
  • Identify as a professional social worker and conduct oneself accordingly throughout all phases of transplant
  • Advocate on behalf of patients for fair and equitable access to services
  • Demonstrate knowledge of community resources and funding possibilities
  • Demonstrate ability to work with multidisciplinary transplant teams in a responsive manner while adhering to professional roles and boundaries
  • Participate in the development or identification of necessary procedures and/or policies that advance the social well being of transplant and VAD recipients
  • Participates actively in departmental patient review activities such as but not limited to: patient selection activities, daily rounds (dept and hospital), financial review meetings, patient education or others as assigned
  • Works closely with Financial coordinator to identify avenues to assist patient with identified needs
  • Understands access to assistance for both transplant specific opportunities and other organization designed to assist patients
  • Works collaboratively with hospital social work and case management in order to assist in identifying patient needs and resolution
