Responsibilities

A successful candidate for this position must possess a valid West Virginia Social Work license. This position is responsible for overseeing the operations and success of our dementia unit, Mary's Garden, ensuring the delivery of high-quality, compassionate care while supporting residents, families, and staff.

The Social Services Specialist works with nursing center patients/residents and their family members/significant others to identify patients/residents' strengths, social, emotional, and mental health needs and then procures the services to meet those needs. This position provides patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, policies and procedures, and standards of care. *Assists with planning and implementing a comprehensive Social Services program.*Works with the interdisciplinary team to promote and protect patients/residents' rights and thepsychosocial well-being of all patients/residents.*Complete a comprehensive Psychosocial Assessment for each patient/resident that identifiessocial, emotional, and psychological needs and strengths. Assesses each patient/resident fordischarge.*Identifies patient/resident discharge goals at admission and documents initial discharge plan andinvolves the patient/resident and family throughout the process.*Educates staff regarding the role of the Social Services in the facility and the psychosocial needsof the patients/residents and their families/significant others including the problems of aging anddisability as requested by the Social Services Director.
