The Senior Medical Director for Primary Care Concierge and Direct Pay Services provides strategic, clinical, and operational leadership for Duke Health's concierge medicine portfolio, including but not limited to Duke Signature Care, Duke Executive Health, Duke Integrative Medicine and weight management services. This leader is responsible for expanding the breadth and impact of service offerings, identifying and developing new opportunities that enhance access, experience, and value for patients seeking premium care options.
This role ensures high-quality, patient-centered clinical care while also driving program growth, operational excellence, and alignment with Duke Health's strategic priorities. The Senior Medical Director will serve as the primary clinical and administrative leader for all concierge service lines. This position requires that the physician provide direct clinical care for 65% effort, and 35% of effort will be dedicated toward the administrative responsibilities of the Medical Director role.
Key Responsibilities
- Program Leadership and Strategic Growth
- Provide executive clinical leadership for Duke Signature Care, Executive Health, Integrative Medicine and other concierge/direct pay service models within the Duke Health portfolio.
- Lead the development and expansion of new primary care concierge and other premium offerings, assessing market and partnership opportunities, designing innovative service models, and aligning with Duke Health's strategic objectives.
- Oversee the creation and refinement of service tiers, membership models, and premium care experience enhancements.
- Ensure programs maintain exceptional patient experience standards consistent with Duke Health's goals and reputation and in alignment with the clinical oversight of the Primary Care Service Line.
- Coordinate and align strategic growth priorities with clinical leadership of affiliated practices in the Duke Health network utilizing similar approaches to care delivery (e.g., One Medical).
- Collaborate with Duke Health marketing and communications teams in studying patient consumerism, analyzing patient decision economics, and reviewing feedback from patients, families, and communities interested in novel care delivery models.
- Collaborate with the Duke leadership to create and help lead a newly enhanced Ambassador program creating a seamless front door for patients seeking concierge and direct pay services
- Clinical Quality, Safety and Experience
- Actively practice in Signature Care, Executive Health , or another Duke Health primary care concierge clinical service.
- Ensure all concierge programs deliver high-quality clinical care and patient-centered services in compliance with Duke Health policies.
- Provide medical oversight and clinical guidance to physicians, APPs, and other clinicians practicing within concierge models.
- Establish and monitor clinical quality metrics, patient experience feedback loops, and continuous improvement initiatives.
- Operational Excellence and Team Leadership
- Oversee Duke Signature Care and Duke Executive Health, offering direct leadership to clinical and administrative staff.
- Partner with operational leaders to ensure program reliability, operational efficiency, and a seamless patient journey.
- Collaborate with finance, marketing, and business development teams to ensure strong performance, sustainability, and service differentiation.
- Engage in committee work or leadership forums as appropriate for program oversight and alignment across Duke Health.
- Innovation and Service Development
- Champion innovation, pilot programs, and service advancement within the concierge and cash?pay portfolio.
- Lead initiatives that support advanced service offerings, coordinated care experiences, and enhanced accessibility for high?touch patient segments.
- Evaluate emerging market trends, technologies, and competitive landscapes to inform program strategy.
- Maintain flexibility to evolve responsibilities as programs scale and new opportunities emerge.
Qualifications
Education
- MD or DO degree from an accredited medical school (required) OR an advanced practice provider with a degree from an accredited nursing or physician assistant school.
- Additional qualifications in healthcare administration (MHA, MBA, MPA, etc.) (preferred)
Licensure/Certification
- Must be licensed to practice medicine in the state of North Carolina.
Required Knowledge, Skills, and Abilities
- Excellent verbal and written communication.
- 7+ years of primary care clinical practice with prior experience practicing in concierge medicine, executive health, or a similar premium care domain.
- Demonstrated experience with program growth, service line expansion, or development of new clinical offerings.
- Proven ability to oversee both clinical and operational functions in a healthcare environment.
- Strong leadership and relationship building skills.

- Strong sense of ethics and integrity in handling sensitive information and making clinical decisions.

Work Experience
- Progressive senior leadership experience in clinical management within a concierge or direct pay primary care or similar model.

- Reports to: Vice President, Primary Care Service Line
- Works in close partnership with operational, financial, and clinical leadership across Duke Health
