JOB ESSENTIAL DUTIES:

Project and Program Leadership
  • Lead the planning, development, implementation, and evaluation of population health projects and strategic initiatives.
  • Develop project plans, timelines, budgets, performance metrics, and operational workflows.
  • Oversee multiple concurrent projects while ensuring deliverables are completed on time and within budget.
  • Evaluate program effectiveness and develop recommendations for continuous improvement.
  • Utilize data analytics and population health tools to identify opportunities to improve patient outcomes and operational efficiency.
  • Ensure alignment of projects with organizational strategic goals and regulatory requirements.
  • Present project updates, outcomes, and recommendations to executive leadership and key stakeholders.

Mammogram Bus Program Leadership
  • Provide operational oversight and strategic direction for the organization's Mammogram Bus Program.
  • Coordinate mobile mammography services, community outreach events, and screening initiatives.
  • Develop partnerships with community organizations, employers, schools, faith-based organizations, and local agencies to increase access to breast cancer screening services.
  • Monitor program utilization, patient engagement, screening outcomes, and operational performance.
  • Identify opportunities to expand mobile health services and improve access to preventive healthcare for underserved populations.
  • Ensure compliance with all regulatory, clinical, and grant requirements related to mobile screening operations.
  • Track and report program performance metrics and community impact outcomes.

Grant Management
  • Research, identify, and pursue federal, state, local, and private grant funding opportunities.
  • Lead the development, writing, and submission of grant proposals and applications.
  • Manage awarded grants from implementation through closeout.
  • Ensure compliance with grant requirements, deliverables, reporting obligations, and budgetary guidelines.
  • Monitor grant expenditures and collaborate with finance teams to maintain fiscal accountability.
  • Prepare grant reports, outcome analyses, and funding updates for executive leadership and funding agencies.
  • Build and maintain relationships with grantors, funding organizations, and community stakeholders.

Preventive Health Programs
  • Develop and oversee preventive health initiatives focused on improving population health outcomes.
  • Support programs related to:
    • Breast and cervical cancer screening
    • Colorectal cancer screening
    • Diabetes prevention and management
    • Hypertension control
    • Vaccination and immunization campaigns
    • Tobacco cessation
    • Nutrition and wellness education
    • Social determinants of health interventions
  • Monitor care gap closure efforts and preventive service utilization.
  • Collaborate with clinical, operational, and community teams to increase patient engagement in preventive healthcare services.
  • Develop strategies to reach vulnerable, underserved, and high-risk population


Quality & Value-Based Care
  • Support organizational quality improvement and value-based care initiatives.
  • Monitor performance related to:
    • HEDIS measures
    • Medicare Stars
    • Medicaid quality programs
    • Accountable Care Organization (ACO) metrics
    • Preventive screening measures
    • Chronic disease management outcomes
  • Analyze performance data and develop action plans to improve quality metrics.
  • Collaborate with providers, care teams, and operational leaders to address care gaps and improve patient outcomes.
  • Support regulatory compliance and accreditation activities related to quality and population health.
  • Lead performance improvement initiatives that drive quality outcomes and financial sustainability.

Executive & Cross Departmental Support
  • Serve as a strategic advisor and resource to executive leadership regarding population health initiatives and community health programs.
  • Prepare executive reports, presentations, dashboards, and performance summaries.
  • Facilitate collaboration across departments including:
    • Clinical Operations
    • Population Health
    • Finance
    • Quality Management
    • Marketing and Community Relations
    • Information Technology
    • Human Resources
  • Participate in organizational committees, task forces, and strategic planning efforts.
  • Support executive leadership with data analysis, project planning, and special initiatives.
  • Assist in developing policies, procedures, and operational standards related to population health programs.


SKILLS, KNOWLEDGE & ABILITIES:
• Strong knowledge of population health and value-based care principles.
• Experience with healthcare quality metrics, HEDIS, Medicare Stars, and quality improvement frameworks.
• Expertise in grant writing, grant administration, and compliance reporting.

Requirements

REQUIREMENTS & PREFERENCES:
  • Bachelor's degree in public health, Healthcare Administration, Nursing, Business Administration, Health Sciences, or related field required.
  • Master's degree in public health (MPH), Healthcare Administration (MHA), Business Administration (MBA), Nursing, or related discipline preferred.
  • Minimum of five (5) years of progressive experience in healthcare operations, population health, community health, program management, grant administration, or value-based care.
  • Minimum of three (3) years of leadership or management experience.
  • Experience managing complex projects and multidisciplinary teams.
  • Demonstrated success securing and managing grant-funded programs preferred.
  • Experience within a Federally Qualified Health Center (FQHC), health system, public health organization, managed care organization, or population health environment preferred.

  • Project Management Professional (PMP)
  • Certified Professional in Healthcare Quality (CPHQ)
  • Lean Six Sigma Certification
  • Grant Management Certification
  • Certified in Public Health (CPH)
