Rutgers, The State University of New Jersey, is seeking a Senior Care Coordination Specialist in the Department of Infectious Disease Practice within the New Jersey Medical School (NJMS).

The Senior Care Coordination Specialist will work as the Licensed Social Work/ Licensed Associate Counselor Mental Health Clinician and contribute to the success of the program by managing the daily workflow to achieve high quality care management delivery and positive clinical outcomes, while meeting the administrative and contractual performance targets, documentation standards and billing requirements of the grant program(s). This position will focus on assessment, counseling, and coordinating care of high risk/high acuity patients with untreated mental, emotional or substance abuse or problems. The LSW/LAC MCM is responsible for meeting the HRSA standards for medical case management that include (1) initial assessment of service needs; (2) development of a comprehensive, individualized service plan; (3) coordination of services required to implement the plan; (4) client monitoring to assess the efficacy of the plan; and (5) periodic re-evaluation and adaptation of the plan as necessary over the life of the client, as well as patient specific health education, counseling, monitoring, assessment and deployment of techniques to assure the engagement and retention in care of the identified patients. LSW/LAC MCM activities include individual and group therapy, crisis intervention, conducting clinical assessments, medical case management, client advocacy, referral, prevention, and education.

Among the key duties of this position are the following:
  • Assess and works collaboratively with the patients, medical provider, designated family/significant others and community support service providers to address the needs identified in the patient's health and psychosocial risk assessment and implement a plan of care based upon the needs identified.
  • Interview clients, review records, and confer with other professionals to evaluate mental or physical condition of client or patient.
  • Conduct and complete a biopsychosocial assessment to include functional status, mental health status, acuity level, prognosis/diagnosis, and assesses the need for treatment services and resources.
  • Performs financial eligibility screening, every six (6) months and assist patients to obtain health coverage if applicable.
  • Facilitates the development and implementation of a comprehensive, multi-disciplinary plan of care for patients with complex mental health and/or substance abuse and socio-economic problems, in collaboration with the physician and other members of the healthcare team.

Minimum Education and Experience
  • Master's Degree in Social Work, Counseling or a related field with a license to practice in the State of New Jersey plus two (2) years post Masters experience.


Certifications/Licenses
