RPN RAI Coordinator
Langstaff Square Community - Richmond Hill

Rate of Pay:$37.68 - $39.35 (as per collective agreement)

Reporting to the Director of Care, the RPNRAI Coordinatoris a critical role within our communities as it impacts the lives of residents.

What you'll be doing:

The RPNRAI Coordinator is responsible for the planning, implementation, coordination, and evaluation of the RAI Program, ensuring compliance with quality management standards, legislative requirements, and organizational policies and procedures.

You will be a welcomed member team and impact in the following ways:

  • Establish, maintain, and communicate current resident assessment schedule to inter-professional team.
  • Ensure RAI accuracy and compliance with coding, capping and care plans are updated.
  • Reviewandanalyzeeachresident'sRAIassessmentstoensureaccuracyandmeet theresidentsneeds.

  • Monitor and analyzes RAI outputs and reports.
  • Produce RAI reports for leadership use to assist in clinical and operational decision-making
  • Responsible for managing the computerized data submission process and maintain accurate records in accordance with Ministry of Health and Long-Term Care (MOHLTC).Collaborate with the leadership team and support services consultants to ensure data integrity and effective data utilization.

  • Monitors and analyzes RAI outputs and reports, and communicates findings with the leadership team and support services committees.
  • Lead the RAI education process for all participating team members


Who You Are:

  • A Registered Practical Nurse in good standing with the Ontario College of Nurses
  • Successful annual completion of RAI certification as per Ministry of Health requirements for Relias-MLTC
  • RAI experience/education is an asset
  • Good problem solving and analytical skills
