Position SummaryReporting to the Team Manager, the RN Case Manager plays a key role in assessing and managing the physical, emotional, social, and spiritual needs of our patients and their families. You will coordinate and implement personalized care plans in partnership with the attending physician, Medical Director, and a dedicated interdisciplinary team. Each RN Case Manager is responsible for a manageable caseload of up to 15 patients and is supported by Revisit Nurses and the Team Manager.
Key Responsibilities- Recognize and respond to signs of active dying and patient transition
- Effectively assess and manage pain and symptoms based on individualized care plans
- Educate patients, families, and caregivers using clear and compassionate communication
- Document all assessments, interventions, and patient progress accurately and promptly
- Deliver holistic, patient- and family-centered care across diverse care settings
- Honor patients' cultural and spiritual beliefs throughout their journey
- Collaborate with interdisciplinary team members and external providers to ensure quality care
- Coordinate care with attention to insurance, authorizations, and cost considerations
- Facilitate care transitions such as General Inpatient (GIP) or Respite when needed
- Supervise LVNs and Home Health Aides with clarity and support
- Participate in team meetings, training sessions, and quality improvement activities
- Maintain compliance with all applicable state, federal, and accreditation standards

Qualifications- Current California RN license
- Minimum of one year of acute care nursing experience
- CPR certification
- Valid California driver's license and reliable transportation
- Experience in hospice or home health (preferred)
- Certification in Hospice and Palliative Care Nursing (highly desirable)
- IV therapy experience (a plus)
- Proficient in basic computer applications and electronic health records
