Responsibilities

  • Lead and oversee a multidisciplinary team of healthcare professionals.
  • Ensure compliance with hospice regulations and standards.
  • Facilitate communication between medical staff and care teams.
  • Organize and document interdisciplinary meetings regarding patient care.
  • Develop and revise patient care policies and emergency protocols.
  • Manage staff scheduling, conduct performance reviews, and cultivate a positive workplace.
  • Serve as the primary communication link with families, physicians, and other healthcare providers.
  • Analyze quality metrics and manage a patient caseload as necessary.
  • Participate in on-call duties and after-hours support for continuous patient care.
  • Enhance resource utilization and foster a collaborative work environment.

Qualifications:

  • Registered Nurse (RN) license with a minimum of 3 years of healthcare experience, preferably in hospice or related settings.
  • Bachelor's degree strongly preferred.
  • Leadership or management experience a major plus.
  • Valid driver's license (required)

  • Ensure adherence to hospice regulations and standards.
  • Facilitate communication between medical staff and the care team.
  • Lead interdisciplinary meetings, documenting patient needs and care plans.
  • Develop and update patient care policies and emergency procedures.
  • Manage staff scheduling, performance reviews, and foster a positive work environment.
  • Act as the central hub for communication with families, physicians, and other providers.
  • Analyze quality metrics and manage a caseload of patients as needed.
  • Participate in after-hours and on-call duties to ensure seamless care.
  • Maximize resource utilization and promote a collaborative work culture.
