Full Job Description
Coordinates the development and deployment of stroke educational plans, communicating updates on outcomes, process changes, and regulatory requirements. Organizes community education events to raise stroke awareness and prevention strategies. Develops and reviews evidence-based clinical pathways to standardize stroke care, ensuring timely implementation and updates. Evaluates stroke program metric performance, ensuring accurate reporting and sharing outcomes with committees and leadership. Rounds on stroke patients to provide education and support, participates in multidisciplinary rounds, and responds to stroke alert events. Conducts periodic follow-up calls to determine patient outcomes and provide additional education and support. Other duties as assigned. Serves as the stroke program clinical content expert, utilizing knowledge of clinical stroke evidence-based guidelines, national accreditation standards, and regulatory requirements. Leads the stroke interdisciplinary committee(s) with the stroke medical director to improve clinical performance, standardize processes, and ensure compliance with standards. Identifies variations in care and develops corrective action plans to improve performance. Leads mortality and morbidity committee reviews to identify opportunities for improving clinical outcomes. Oversees the initial and ongoing national accreditation process, including readiness rounds, mock stroke alerts, education, and documentation maintenance. Acts as the stroke program lead during accreditation site visits, collaborating with campus leadership to maintain accreditation.
