What You'll Do: - Perform resident assessments and monitor changes in condition
- Develop, implement, and update individualized care plans
- Administer medications and treatments
- Collaborate with residents, families, physicians, and interdisciplinary teams
- Educate residents and families on care plans and health management
- Ensure compliance with safety standards, policies, and resident rights
- Accurately document care, incidents, and changes in condition
RN Qualifications: - Current and active Pennsylvania RN license
